Optional Practical Training (OPT) Instruction
The F-1 OPT Application Checklist is here.
Instructions: For Optional Practical Training (OPT), please meet with an ISSS advisor with the following:
- Completed Academic Advisor’s Recommendation Form
- Completed Unemployment Termination Policy Form
- Completed Not Responsible for Any Loss or Delay of OPT Application Form
- Completed Understanding Travel Risks While on OPT Form
- Copies of your current and previous DS-2019(s)
- Copies of passport pages (biographical data, photo, and expiration date)
- Copy of your current 1-94 card (front and back)
- Copy of any previously-issued Academic Training authorization letter(s)
After all OPT forms and copies of your immigration documents are electronically submitted to ISSS, please contact ISSS at isss@gallaudet.edu to schedule an appointment.
